1. CompuServe's Slam Dunk Contest is a skill-based contest for the regular customers over 18 years of age of the CompuServe Information Service. Employees of CompuServe Incorporated, H&R Block, CompuServe Information Providers, SysOps, GameOps, American Express Travel Services, their affiliates, subsidiaries, advertising agencies, and immediate families are ineligible to participate and win prizes. This contest may be discontinued at any time at the sole discretion of CompuServe Incorporated. The contest is subject to all local, state, and federal regulations. The contest is void in the Province of Quebec and wherever elseprohibited by law. All taxes are the sole responsibility of the winners.
2. CompuServe's Slam Dunk Contest is open for entries beginning 12:01 Eastern time on 28 February, 1995. Only one entry per CompuServe ID will be accepted. Entries will be accepted through 11:59 p.m. Eastern time on 3 April, 1995.
3. To enter, a member must correctly answer a series of five multiple choice trivia questions. A player will have two chances to answer each question correctly. All players who correctly answer all five questions will be entered in a random drawing for the Grand Prize of a trip for two to the NBA Playoff game of your choice and for the Official NBA Team Yearbooks.
4. Winners will be selected in a random drawing on Monday 3 April, 1995. The Grand Prize winner will be notified by telephone and certified mail. All other winners will be notified by CompuServe Mail. Winners' user Ids will also
be posted online in the CompuServe Slam Dunk Contest database. The decision of the judge(s) is final and not reviewable by any other person, agency, or tribunal. 5. The Grand Prize is a trip or two to the NBA Playoff Game of your choice. The prize includes roundtrip coach airfare from any US city plus two tickets to the game. Retail value of the Grand Prize is $750. 850 Official NBA Team Yearbooks will also be awarded. The retail value of the team year books is $9.95. No prize may be exchanged, substituted, modified, or redeemed for cash. One prize per family, individual, household, or User I.D. Number. Prizes are not transferable. [Prizes won by entrants under the age of 18 will be awarded to the winner's parent or legal guardian.] All prizes will be awarded and mailed to the winner's address contained in CompuServe's online records. By acceptance of their prizes, winners consent to publication of their names, likenesses, and/or User I.D. without further compensation for advertising or promotional purposes. Prizes not claimed after 21 days of the date of notification, for any reason whatsoever, will be forfeited. Prize winners may be required to execute an affidavit of eligibility and publicity release within 21 days of notification. Non-compliance within this time period will result in disqualification and an alternate winner will be selected if possible.
6. USA prize winners will be required to provide CompuServe with their Social Security numbers by completing Federal Tax Form W-9 which will be mailed to all winners. Prizes valued at $150 or more will not be distributed until the completed Form W-9 is received by CompuServe. Form W-9 will be distributed with prizes valued at $149.99 or less. No responsibility is assumed for lost, misdirected, or late mail. For winners residing in the United States, the value of the prizes won in the Slam Dunk Contest will be reported by CompuServe on Federal Tax Form 1099 for each individual awarded a total retail value of at least $600.00 in CompuServe Information Service prizes during tax year 1995. International winners will be required to answer a time limited mathematical skill testing question.
